Give the family something real before they leave the ward.
You have forty-eight hours, a family in shock, and a photocopied list of agencies with no prices on it. This is the alternative, and it costs your hospital nothing.
How it works on a ward
No login, no account for the family, nothing for you to administer.
- 1
Four questions
Discharge date, postal code, what the family needs help with, and roughly how many hours a day. About forty seconds.
- 2
Confirmed availability only
We show only providers who have confirmed they can start on that date — 5 of our listings can start within 48 hours today.
- 3
The family decides who calls
Prices are on the card before anyone is contacted, and no provider gets a phone number until the family ticks their name.
Why a hospital can hand this out
Discharge planners are constrained in what they can recommend, and rightly so.
We are not a referral agency
We take no fee when a family chooses a provider. Providers pay a flat monthly listing fee and nothing else, so there is no placement incentive for anyone to disclose.
Nobody is recommended
The tool shows every provider who can meet the family's date and needs, ranked by a published formula, with prices. It does not advise.
No data flows to you or from you
Families use it themselves. Your unit does not enter patient information, hold an account, or become a processor of anything.
Every listing is verified
Licence, insurance and vulnerable-sector checks, re-verified every six months. A lapsed licence hides the listing the same day.
What we can set up for a unit
- Printed cards for the ward with a QR code to the discharge tool
- A short link for your unit so we can tell you how many families used it
- Printed copies of the 72-hour discharge checklist
- A named guide your planners can phone when a discharge is complicated
