Arranging care for a parent in India while you live in Canada

What can be organised from 12,000 km away, what cannot, and how families usually structure it.

The short version

  • A local named coordinator matters more than the agency brand.
  • Elder care in India is largely unregulated — verification has to be done by someone on the ground.
  • Expect to pay in INR; multi-currency payment adds a 2–4% spread unless you plan for it.
  • Time-zone-aware reporting is the difference between reassurance and 2am phone calls.

The first decision is who holds the file

Families in Toronto managing a parent in Chennai or Pune fail at the same point: nobody local owns the arrangement. Agencies change staff, neighbours mean well but are not accountable, and siblings in three time zones each assume another one is handling it.

The structure that works is a single named care coordinator in the city where your parent lives, paid to be accountable, with a monthly written report and photos.

What can be arranged remotely

Attendant care at home, nursing visits, physiotherapy, hospital escort, pharmacy runs, bill payment and pension life-certificate paperwork can all be arranged and paid for from abroad.

Emergency response cannot be arranged from abroad. Someone within thirty minutes' drive has to be nominated, told they are the nominee, and given a key.

Money

Providers invoice in INR. Paying from a Canadian card usually means a 2–4% spread you did not see quoted. Setting up a local account your coordinator draws against, with a monthly cap and a statement, is cheaper and more auditable.

Agree in writing what is included. Attendant care in India is often quoted as a 12-hour or 24-hour rate with food and accommodation assumptions attached, which is a different shape of contract from a Canadian hourly rate.

Keeping siblings aligned

Write down who does what: who is the coordinator's contact, who pays, who attends the annual medical review, who visits and when.

A shared monthly report with photographs stops the slow erosion where one sibling carries everything and the others hear only about crises.
